Nursing Funding Checklist

How to Get Nursing Care Approved Under the iBudget Waiver

When your loved one is declining, the paperwork often has not caught up. Answer a few questions about care needs, never about identity, and get the documentation needs like these are usually built on, who writes each piece, and the order to request them in.

Where things stand

Where does your loved one live right now?

This changes who you ask for what, not whether you can ask.

Is nursing authorized in their plan right now?

Most families have never been shown this in writing. Not sure is a completely normal answer and it is useful on its own.

Compared with twelve months ago, how much care does your loved one need?

This is the question almost nobody thinks to answer, and it drives everything that follows.

Optional

What does their current authorization actually say?

If you have the support plan or a service authorization in front of you, type what it lists. Residential Habilitation at a level such as Standard, Moderate, Extensive 1 or Extensive 2. Personal Supports hours. Skilled nursing hours per day or per week. Companion. Behavior Analysis. If you do not know, that is an answer too, and it is the first thing to ask your coordinator for.

Please leave out names, dates of birth, and Medicaid numbers. Nothing you type here leaves your browser unless you choose to send it to us.

Optional

What does the QSI say about them?

If you have the Questionnaire for Situational Information, the wording matters more than the scores. Type or paste what it says about their medical and behavioral needs. That language is what a reviewer reads, and it is usually where the gap is most visible.

Please leave out names, dates of birth, and Medicaid numbers. Nothing you type here leaves your browser unless you choose to send it to us.

What has happened in the past year

Check anything that happened, even once. Each of these leaves a paper trail you can request.

Optional

In your own words, what is different now?

Compared with a year ago. Write it the way you would say it out loud to someone who knows your loved one. This is the part no checkbox captures, and it is usually the part a physician finds most useful.

Please leave out names, dates of birth, and Medicaid numbers. Nothing you type here leaves your browser unless you choose to send it to us.

Which areas should we look at?

Check any area where things have changed or feel hard. Only the areas you check will open up with more detailed questions.

Optional

Has anyone told you something cannot be covered?

If someone has said no, or said it is not available, write down what they said as closely as you can remember. The exact wording often changes what you should ask for next.

Please leave out names, dates of birth, and Medicaid numbers. Nothing you type here leaves your browser unless you choose to send it to us.

This checklist is for document preparation and education. It is not clinical advice and it is not representation. If you are appealing a decision, your waiver support coordinator, your own attorney, or the state's designated advocacy organization can help you with that process.
